Volunteer Requirements: TB Tests and Prints
To become a volunteer at Carnegie Middle School, as well as any other school in the district, you must be fingerprinted and have a current TB test on file with your school, per SJUSD Board Policy.
Also, any volunteer who will be handling money for fundraisers or field trips must be also fingerprinted for SJUSD.
Fingerprinting
If you have never been fingerprinted with our district before, pick up a form from our office. Fingerprinting is free, and can be done at our district office on Walnut Avenue daily. Once you are printed, your clearance is good as long as you have a student in the district (and commit no crimes).
Note: fingerprint clearances are considered confidential information and thus are not transferred from school to school. So, as your child switches schools, you must contact each new school so they can request their own copy of the clearance.
If you have been fingerprinted at your previous school, please call the Carnegie school secretary, Ann Silberman (971-7853), and give her your full legal name, your birth date, and the last four digits of your social security number. Your clearance will be transmitted to Carnegie from DOJ.
TB Testing
All parent volunteers are also required to have a current TB test clearance.
There are several ways to do this:
You can go to the District Office on Walnut on Mondays, Wednesdays, and Fridays from 3:30 to 5:30 to have your test done. Please be aware that the test is done in two parts - one for skin prick and one for reading, so give yourself those days. (Reading only on Fridays.) When you get your results, you must bring them to the office yourself. SJUSD will not release this information to the school sites.
You can also go to your own physician and bring a copy to the office. The Doctor's Center on San Juan also does TB testing for a nominal fee.
TB tests are good for two years.
Driving Forms
If you are going to be driving on a field trip, you must have a current insurance form filled out. They are in the office. We will also require a copy of your driver's license and insurance card. These things must be turned in yearly and/or any time the information changes. The information is shredded at the end of the year.
We realize that these requirements are inconvenient for many parents, but the bottom line is this is for the safety of all the students. We would urge you to take care of this early in the year so that you will be free to volunteer at the school as much as you like.
