Payroll Forms

stamp The Payroll Department plays an integral role in the life of every employee. Make sure that Payroll has the latest and most accurate information about you and your employment status. The following forms can help you stay current with Payroll.

If you have further questions, please call Payroll at 971-7223 and follow the telephone tree or stop by the Payroll office during normal business hours

Stamp Sign up for Direct Deposit

All employees are highly encouraged to use Direct Deposit. Direct Deposit ensures your money is in your account on time. To authorize Payroll to send your check directly (electronically) to your bank or credit union:

Note: It takes two months for this to become effective. You should get one live check and the next one will be deposited in your bank.

Stamp Cancel Your Direct Deposit

If you should need to cancel your Direct Deposit:

Stamp Employee Absence Report

When time is taken off work, absence forms must be submitted to Payroll no later than three business days upon return from your absence. Absence forms may be submitted to Payroll without the employee's signature, but the absence form MUST have been approved by a supervisor.

Stamp Change Your Address

Many times during the year, the district office may need to send you information, check payments or W-2's and if your address is wrong, you will not receive the correspondence. Live checks will NOT be forwarded by the Post Office. To update your address or phone number:

Stamp Change Your Payroll Withholding

All new employees should fill out a W-4 (Federal Withholding Form). Your federal and state taxes will be based on your marital status and the exemptions you claim. Upon hire, if payroll does not have a W-4, your Federal and State withholding status will default to Single with Zero exemptions. When you want to change your withholding for Federal or State Taxes, you must fill out the appropriate form. Payroll can tell you how much is currently being deducted.

Stamp Understanding Your Pay Stub

This file is a visual explanation of the many boxes on your pay statement.

Stamp Sign-up for San Juan Education Foundation Payroll Deduction

If you wish to support the San Juan Foundation by a payroll deduction, this form is used to initiate, change or cancel a deduction from your salaris/wages:
