Employee Benefit Open Enrollment for 2015-2016
begins November 9th and ends on November 20th.

Welcome to Employee Benefit Open Enrollment. Please refer to the links and documents below to obtain important information about your health benefits. This is a good time to review your health plan selections. If you are adding dependents, you are required to provide proof of eligibility as follows: marriage certificate, copy of front page of federal or state tax return (financial information may be redacted),  birth certificate, domestic partner registration, court document for adoption or legal guardianship.  Please note the hearing aid benefit is a maximum of $1,000.  The open enrollment flyer inaccurately states the maximum to be $1,500.

Employee Benefits Information - Annual Section 125 Enrollment

Employee Benefit Open Enrollment is the time when employees must enroll or re-enroll in our Section 125 Reimbursement Accounts which are administered by American Fidelity.  Employees, who want to participate in a pre-tax reimbursement account for dependent care or medical expenses, must enroll each year to continue this benefit.
