Form must be filled out by parent/guardian annually, to authorize participation in our swimming program.

Form allows school staff to administer medication at school. Form must be updated annually, and signed by a physician before any meds. can be given at school.

1. chronic illness verification form
   This form, signed by a physician, documents the chronic illness diagnosed for a student that may lead to frequent absence. This form allows the parent to verify illnesses, by writing to the school in lieu of acquiring a doctor's note for each absence. This document expires at the end of the academic year it was received.
