Field Trip Requirements & Forms

Mission Ave. Open School participates in many field trip experiences.  Parents who plan to volunteer to drive on field trips must file a vehicle usage form with the front office.  Those who plan to attend overnight field trips are required to be fingerprinted and be tested for TB.  These forms and information will be uploaded in the near future.  The field trip forms teachers use for the field trips are available on-line too.

If you have installed Adobe Acrobat Reader 8.x, you will be able to fill out fields in the form (but not save the file with your changes) with the information provided by your teacher and print it out at home.  In the near future, the teachers should be uploading these forms for each trip and you can download it & print it at home, if you should need a copy.  If you have not or cannot download the latest version of the Acrobat Reader, you can still download this file, print it and fill it out by hand.
