District Forms
You MUST complete the FINGERPRINT form and/or the VEHICLE USE form if you plan to: - Chaperone ANY band event,
- Ride the bus with students on field trips, or
- Use your own vehicle to transport students.
If a situation occurs where we need parent drivers/chaperones and they are not cleared, the school district will cancel our activity. FINGERPRINTS If you plan to volunteer for the band in ANY capacity, you MUST be fingerprinted. -
Fingerprinting is free through the school district. If you choose to be fingerprinted outside the district, you will be charged a fee by the agency that takes your fingerprints. -
Form is available online--see below. -
Fingerprints do not expire, unless the clearance is revoked by the Department of Justice. -
If you were already fingerprinted for El Camino, email Mr. Glaser at kglaser@sanjuan.edu so he can notify the office. -
If you were printed at another school, email Mr. Glaser at kglaser@sanjuan.edu. He needs to get some basic information from you so you can be put on the El Camino list. VEHICLE USE FORMS - These forms stay on file at El Camino.
- They must be updated each time the driver's insurance policy expires.
- Form is available online--see below.
- If you have already completed a form THIS YEAR for El Camino, please email Mr. Glaser at kglaser@sanjuan.edu.
PERMISSION SLIPS & MEDICAL RELEASE FORMS These forms allow students to attend specific events and receive medical attention in case of an emergency. These forms must be on file for all students participating in band activities. If a serious medical problem arises, parents or guardians will be notified immediately.
