    Transition
    This web page will serve as a central resource for SJUSD staff, students and families for information regarding transition planning and instructional support. I would appreciate suggestions for resources and info that should be added to this page. 
     California and federal legislation mandate that transition services are included in a student's Individualized Education Program (IEP) by the time the student turns 16.
     
    Determining transition services is a collaborative task for the entire IEP team including the student and parents.
    The core post-secondary goals (outcomes) that must be addressed in the transition plan include:
      1.) post-secondary education/training,
      2.) employment, and as appropriate 
      3.) independent living.
