• The District provides a term life insurance policy based on employee benefit eligibility status. The amount of coverage varies by employee group. All benefit eligible new hires need to designate a beneficiary for the term policy in the AfEnroll benefit platform within 30 days of hire.  You may change your beneficiary designation at anytime, by updating the AFEnroll benefit platform.  

    Need Supplemental Life Insurance?

    SJUSD pays for your basic life insurance, however, you and/or your dependents may need additional life insurance to assure your family is secure should something happen.  You can purchase an additional $25,000 or $50,000 without having to submit Statement of Health form if you apply for this insurance within 30 days of your date of hire.  Policies over $50,000 does require the employee to complete a Statement of Health form which the employee sends to the insurance carrier for approval.   You may obtain the Statement of Health form based on your employee group below:
    You may view the premium costs for the additional coverage on the Benefit Cost Schedule tab to the left.
    You may also purchase a $1,500 or $10,000 life insurance policy for your spouse and/or dependent children(up to age 21 or age 25 if a full-time student) within 30 days of your benefit eligibility.  You would be the beneficiary for the  Supplemental Spouse/Dependent Life policy.  If you wish to enroll your spouse outside the initial 30 days of benefit eligibility, you may apply for spouse coverage by completing a Statement of Health form applicable to your employee group.
