• How-to Verify Employee Vaccination Status

    Step 1

    Using any device with an up-to-date web browser, visit https://portal.sanjuan.edu and log in using your standard district login. 

    Your username is your full email address: joe.smith@sanjuan.edu or jsmith@sanjuan.edu. If you do not know your password, click the “Can’t Access Your Account” link and follow the prompts to reset your password. 

    You can use a cell phone, tablet, Chromebook, or computer. (Using a phone or another device with a built-in camera may make uploading images of documents easier.


    Step 2

    Once inside Portal, find and click the Verify COVID-19 Vaccination/Testing tile:

    Step 3

    You’ll see another login screen. Use your district login credentials again, but this time your username is only the FIRST part of your email address: joe.smith or jsmith. Enter your credentials and click “Sign In”

    Step 4

    Select the “Verify your Vaccination Status” option from the main menu.

    Step 5

    On the verification screen, your name and work email address will appear and cannot be changed. Enter the phone number that is best to reach you if there are questions about your submission.

    Step 6

    Carefully review the definition of fully vaccinated that is provided on the screen. Then, answer yes or no to the question, “Have you been fully vaccinated?”

    If you answered YES, proceed to step 7. 

    If you answered NO, press the submit button. You will be given information regarding required testing. You’ll also be emailed a copy of the information at your work email address. Starting the week of Oct. 11, you’ll need to return to the system each week to verify your test results. 

     


    Step 7 

    If you answered “YES” in step 6, you will be prompted to enter the date of your final vaccine dose which can be found on your vaccination record. Select the calendar icon in the field to select the date of your final dose.

    Step 8

    Your final step is to upload an image of your official vaccination record. (Acceptable types of vaccination records.)

    To upload your record, click the “Browse…” button.

    If you’re using a computer or Chromebook, you will be given a window where you can locate your record’s file and select it to upload. You can upload a .JPG, .PNG, or .PDF document. 

    If you’re using your mobile device that has a camera, you may also have the option to select your camera and take a photo of your record. Alternatively, you’ll be able to select a file saved on your mobile device to upload. The process may vary depending on your device. 

    Once you have selected your file or taken your photo, click the upload button.

    Once your file is uploaded, the file name will appear on the screen. If you upload the wrong file you can click the “remove” link to remove the file.

    Step 9

    Click the submit button.

    You’ll receive a confirmation message on the screen. Read the information presented and you are done. 

    Please watch your email carefully for the next few days. Your submission will be reviewed. If it is approved, you will receive an email confirming that it has been accepted and that you have met the requirements of the state health order. If your submission is not approved, you’ll receive an email alerting you to the reason why and letting you know that you’ll need to test on a weekly basis until you submit verification and it is approved.
