Accessing Your Digital Vaccine Record
Employees and volunteers can verify their vaccination status by providing a copy of their vaccine record.
If you don't have your record, you may be able access a digital copy through California' Digital COVID-19 Vaccine Record system.
1. Visit https://myvaccinerecord.cdph.ca.gov/
2. Provide the requested information including your name, date of birth and a mobile phone number or email address that would be associated with your record. You'll also need to enter a 4-digit PIN. This can be any 4-digits you choose but keep it handy as you'll need it to access your record.
3. After submitting your information, you'll receive a text message or email with a link to your record if one is found. Click the link and enter the requested information including the PIN you set in step 2.
Your record will be displayed like the one below. Use the options presented to save it to your device or take a screenshot that you can upload to verify your vaccination status with San Juan Unified.
If your record is not found, it is likely your vaccination provider used different information. Verify the phone number, email address and date of birth you're submitting and check to see if it matches what your provider has on file.
