• All coaches, paid or volunteer, are required to submit a completed application packet and certifications, as noted below. The application process is started by contacting the school site Athletic Director.

    • SJUSD application packet: This is obtained from the Human Resources department after you receive your approval forms from the school site Athletic Director
    • Current CPR and First Aid or CPRFPR (For the Professional Rescuer) and First Aid.
    • For aquatics coaches - CPRFPR and Lifeguard certification (which includes first aid) or Water Safety for swim coaches and current CPR/FA.
    • Tuberculosis screening: (within 60 days of beginning coaching assignment) Tests can be done by your own medical office or by the San Juan Unified District Office. The SJUSD screening office is located at 3700 Garfield Ave. Carmichael, 95608. For more information on TB screening, call 916-971-7643.
      • TB screening is done at the SJUSD Screening Office on Monday, Wednesday, and Friday - 3:30 p.m.-5:30 p.m..
    • Fingerprint verification - See your site Athletic Director to obtain a form giving approval for you to fingerprint. Cost is $78, check or money order ONLY (made out to San Juan Unified School District or SJUSD). Fingerprinting services are free for volunteers.
    • Signed Code of Ethics/Undue Influence form
    • Psychology of Coaching Class and Examinations, American Sports Education Program (ASEP).
    Applications are available from Athletic Directors. Submit coaching applications and certification to the Athletic Director where you are applying to coach.
