Please submit ONE form per student. Do not submit multiple requests for your student. Failure to provide valid ID with your submission will delay the withdrawal process. Only parent/legal guardians can withdraw students (no stepparents, other other family members). Former students over the age of 18 must request their own records.

- Report Cards TK-8th grade
- 504 Plans
- Health Care Plans
- Whitehouse Therapy-related records
- SSID Numbers
Note- cumulative files and hard copy records of in-district students are kept for 5 years then destroyed. Not all records are digital. If the student left the district, please contact the last known school of enrollment for records.
Please be advised that we need at least five (5) working days to prepare your request.
Academic-related Education Records are kept for five years past the last date of enrollment. At the time of archival expiration, the records are destroyed.
California Education Code (Cal. Code Regs. Tit. 5, § 437 - Retention and Destruction of Pupil Records, Retention and Destruction of Records) and the compliance thereof which requires records be kept for at least 2 years; San Juan Unified School District maintains the Academic Records of students five years past the last date of enrollment.
