    To become a volunteer at Arcade Fundamental Middle School, as well as any other school in the district, you must fill out a volunteer form and be fingerprinted with our school, per SJUSD Board Policy.

    Please pick up a fingerprint clearance form from our office if you have never been fingerprinted for the district. Fingerprinting is free, and is held at our district office by appointment. volunteers schedule their own appointment by going to San Juan Unified School District's public website : www.sanjuan.edu/Operations/HumanResources/Fingerprinting and Livescan Requests.  Monday - Friday from 9:00am-1:30pm. Once you are printed, your clearance is good as long as you have a student in the district (and commit no crimes!). Please Note: Fingerprint forms MUST be picked up in the school office. The District office will not fingerprint you if the form does not have Arcade's Principal official signature.

    Note: fingerprint clearances are considered confidential information and thus are not transferred from school to school. So, as your child moves up, you must contact each new school so they can request a copy of the clearance.If you have been fingerprinted at your previous school, please contact Michele Ebner 971-7300, michele.ebner@sanjuan.edu  in the Arcade office , and give her your full legal name, your birth date, and the last four digits of your social security number. Your clearance will be transmitted to Arcade from DOJ within a few days.

    Driving Forms

    If you are going to be driving on a field trip, you must have a current Volunteer Personal Automobile Use form filled out along with a photocopy of your valid California Driver's License License and a photocopy of your Insurance Policy Declarations page.. They are in the office if you need one, or you can download it here. These must be renewed each year.

    We realize that these requirements are inconvenient for many parents, but the bottom line is these rules are for the safety of all the students. We would urge you to take care of these things early in the year so that you will be free to volunteer at the school as much as you like.
