School Funding Form

Schools may collect student income data in one of the following ways:

1.   Encourage families to complete the School Funding form in the Parent Portal (Q.)

2.   Use hard copy translated PDF forms, available here: School Funding Form PDF Translated Send the completed forms, through district mail, to the OSLA office and OSLA staff will input information into the system. It will take a week for the information to reflect in the system.

3.   Have parents complete the information using this link to a simple jot form: School Funding Form Digit

4.   Call families and input information into the phone log, accessible here: School Funding Phone Log 22-23al Link Jot Form

Families need to fill out the form once and it will populate to all of their students.
