San Juan Unified is partnering with UC Davis Health to provide COVID-19 vaccination clinics at two locations in the district:
- Sylvan Middle School every Thursday (except April 14) through the end of April from 1:30 to 6 p.m. in the school’s library
- Arcade Fundamental Middle School every Tuesday in April (except for April 12) from 3 to 7 p.m. in the school’s MP room
Pfizer doses will be provided for all eligible ages (including students, family members and community members). Individuals will receive a $25 gift card if they receive their 1st or 2nd dose at one of these clinics.
Walk-ins are welcome, but appointments are preferred:
A consent form is required to be filled out by a parent/guardian for children under the age of 18.

San Juan Unified is once again partnering with Rite Aid to provide a COVID-19 vaccine booster clinic for students and staff on Thursday, March 3 at San Juan Central, located at 3700 Garfield Avenue in Carmichael.
Appointments are available on a first-come, first-served basis and registration is required. Please visit www.sanjuan.edu/booster to sign-up for this opportunity.
Students must be accompanied by a parent/guardian. All participants are asked to bring:
- A record of their first and second doses
- A form of identification
- A copy of their health insurance card, if available
- A completed consent form

Feb. 10, 2022
San Juan Unified is partnering with Rite Aid to provide a COVID-19 vaccination clinic for San Juan Unified students and staff. The vaccination clinic will be held at San Juan Central on Thursday, Feb. 10 from 2 to 5 p.m. Individuals can sign-up to receive their first dose (must return on March 3 for the second dose) or a booster shot.
Appointments are available on a first-come, first-served basis and appointments are required. Visit www.sanjuan.edu/Feb10vaccine to make an appointment.
Students must be accompanied by a parent/guardian. All participants are asked to bring a form of identification and a copy of their health insurance card, if available. Consent forms are required and attendees are encouraged to bring the completed consent form with them to expedite their appointment. If signing up for a booster shot, participants must bring a record of their first and second doses.

San Juan Unified is partnering with Rite Aid to provide a COVID-19 vaccination clinic for students ages 5 to 11 years old. The first dose vaccination clinic will be held at San Juan Central on Wednesday, Nov. 17 from 1 to 5 p.m. Students receiving their first dose on Nov. 17 will need to return to the same location on Dec. 8 (between 3-7 p.m.) for their second dose. The Pfizer vaccine will be distributed at this clinic.
Appointments are available on a first-come, first-served basis and appointments are required. Visit www.sanjuan.edu/Nov17vaccine to make an appointment.
Students must be accompanied by a parent/guardian. All participants are asked to bring a form of identification and a copy of their health insurance card, if available. Consent forms are required and we encourage you to bring the completed consent form to expedite your appointment.
Parents/guardians are strongly encouraged to make individual appointments with their child’s healthcare provider or an individual medical practitioner to discuss the vaccine for this age group.

We will once again be partnering with Rite Aid to provide COVID-19 vaccination clinics for anyone 12 years and older as we prepare to head back to school. Clinics will be held at San Juan Central on the following days and times:
- Tuesday, Aug. 3 from 1 to 5 p.m. (second dose on Aug. 24); sign up at www.sanjuan.edu/August3vaccine
- Thursday, Aug. 5 from 1 to 5 p.m. (second dose on Aug. 26); sign up at www.sanjuan.edu/August5vaccine
